NOTE REGARDING 3 TOP SPONSOR LEVELS: Due to publication deadlines, this form must be received no later than December 1, 2012 for your logo to be included in early publications and printed items. See below, how to send your logo. $2500 - $5000 Supreme Chocolatier* Sponsor Level - Reserved Table, Eight (8) tickets to event, top recognition as event sponsor and in the program, logo* and acknowledgement on all advertising materials including thank you ad; verbal recognition during event; your banner displayed at the event. $1000 - $2000 Royal Truffle Sponsor Level - Reserved Table, Six (6) tickets to event, recognition in the program, logo* and recognition on all advertising materials including thank you ad; your banner displayed at the event. $500 - $750 Chocolate Marble Sponsor Level - Four (4) tickets to event and recognition in the program. $250 Chocolate Cupcake Sponsor Level - Two (2) tickets to event and recognition in the program. $100 Chocolate Chip Sponsor Level - One (1) ticket to event and recognition in the program.
